Services:

  • Home visits, classes for families with young children. Services are provided by community health nurses and community health workers.
  • Clients served are high/at-risk pregnant & postpartum women, high/at-risk infants, infants and children with special health care needs.
  • Classes available are childbirth, parenting, community resources and nutrition.
  • Specialized teams and partnerships are based on community needs and funding, and include ADAPT focusing on pregnant and parenting women in criminal justice system, Healthy Birth Initiative focusing on reducing perinatal disparities among African Americans in NE Portland, Nurse Family Partnership focusing on first time pregnant women, and Healthy Start, focusing on first-time parents.

Services: Nurse Family Partnership is a program for women and their families who are expecting their first child. Each mother we serve is partnered with a registered nurse early in her pregnancy and receives ongoing home visits that continue through her child's second birthday. This is an evidence-based program whose goals include improving pregnancy outcomes, improving child health and development and improving the economic self-sufficiency of families.

Services:
  • Manage Multnomah County's effort to achieve the statewide Healthy Start goal to create a system of support for all first time parents in Multnomah County.
  • Key services are universal prenatal or at birth screening of all first births to insure early identification of those families who may need support and providing that support in a timely and effective manner.
  • Welcome Baby hospital visits for first time parents and teen parents are offered at local hospitals, assessing need and providing parents and teen parents are offered at local hospitals, assessing need and providing parents information on community resources.
  • For parents wanting more parenting education and support, home visit services are offered through Health Department Early Childhood teams or through contracted community based Healthy Start Family Support teams and Teen Family Support teams.

Services: Purpose of this initiative is to reduce disparities in perinatal health among African American women in Multnomah County. Services include home visits, education, and support groups from pregnancy through the child's second year of life. A community mobilization intervention model works to involve the community in improving health outcomes.
